The Acute Family
Medicine Clinic Online Privacy Policy has been developed as an extension
of our commitment to combine the finest quality care and service with the
highest level of integrity in dealing with our patients. This Policy
guides how we collect, store and use information obtained through our
website, and it will be periodically assessed against current business
practices and our patients' changing needs.

to reflect legal, technological and other developments. In that event the

Our Policy
recognizes two kinds of personal data. Personally-Identifiable
Information includes, for example, e-mail addresses. The second
category, Sensitive Data, requires additional safeguards. Sensitive
Data includes, by way of example, Social Security numbers, personal
financial data, and information about a patient’s specific medical
condition.

Collection.
Personally-Identifiable Information will only be collected to the extent
that the Acute Family Medicine Clinic (AFMC) deems reasonably necessary
to serve a legitimate business purpose. Please be aware that if you
provided your email address through our Grand Opening Advisory system,
you will in the future be receiving email from us pertaining to our
grand opening, and may receive additional informational emails from time
to time thereafter. It is our strict policy that email addresses
collected through this system will never be sold or given to any party
outside of the AFMC, for any reason.

Sensitive Data:
Data including our patients’
names, physical or mailing contact information, medical conditions and
treatments will not be stored or maintained at any time or in any
fashion on our domain web server. Patients should not include any such
Sensitive Data in any email to AFMC, as our email server does not
provide encryption for the purpose of transmitting confidential or
private correspondence.

Visitor Tracking:
We do not have in place, nor do
we anticipate implementing any technology to track movement within or
otherwise identify visitors to the AFMC website.

Opt Out.
If you have previously provided your email address through our Grand
Opening Advisory system, and do not now wish to receive emails or
information from AFMC, you can be permanently removed from the system at
any time by emailing our online Customer Service department at:

Personally-Identifiable Information:
You can additionally correct, update, or change the email address you
have previously provided by emailing:
customerservice@acutefamilymedicine.com. If your change request is
sent from an email address other than that you previously provided,
please include the original email address in your request.
