We require photo identification such as a driver’s license, military I.D., or passport for every visit. We will photocopy the ID card and your insurance card for your record.

Treatment of Minors

A parent or legal guardian must accompany a minor patient, except in those circumstances exempted under law (Alaska Statutes), for an initial visit. The parent or legal guardian may designate someone else to accompany the patient if it is specified on the Minor’s Consent for Treatment Form.
